Health and wellness are an ongoing journey that can sometimes be difficult to manage. That’s why our Health Home Care Management Program has built a support network to help children and families navigate their medical, social, educational, and mental health needs. 

Our dedicated care managers work directly with families and their healthcare professionals. This close, personalized support helps ensure that special attention is paid to family’s needs when no one else can give them the answers that they are looking for.

We offer:

  • Individual and Family Support Services
  • Transitional Care Support Services
  • Comprehensive Case Management Services.
  • Education and Social Support Advocacy
  • Assistance with Community Resources
  • Access to the Home and Community Based Waiver Services (subject to eligibility approval)

 

At Seamen’s Society, we have a uniquely talented team with broad-based skills to support children and families in Brooklyn and Staten Island. This includes experience working with individuals diagnosed Complex Childhood Trauma, or Serious Emotional Disturbance, as well as those who have been diagnosed with two or more chronic conditions such as asthma, diabetes, obesity, teen alcohol abuse, or substance abuse.

At the core of our approach to supporting children and families are voice and choice. In other words, we believe that individuals need to be heard by their healthcare providers and have a clear understanding of all of the options available to them to make an informed decision regarding their own care, or the care of a loved one. But we know that this can be daunting without an advocate standing by you — which is where Seamen’s Society steps in.
